Documentation Compliance Services

The AccuDOCUMENT Documentation Compliance Program provides education and training for Clinical Documentation Specialists to ensure physicians document relevant diagnoses and procedures leading to correct MS-DRG assignment.

With the expansion of the familiar CC classification to include CCs and major CCs (MCCs), physicians will be required to provide coders more specific information than was necessary. Considering this total revision of the CC/MCC structure, along with documentation and coding adjustments being implemented October 1, 2007, it has become mandatory for hospitals to engage medical and coding staff in a clinical documentation improvement program.

The objectives of the program are:

  • To improve medical record documentation at point-of-care to ensure quality of care, compliance, and appropriate reimbursement;
  • To promote teamwork in Hospital and Medical Staff personnel engaged in patient care and medical record documentation; and
  • To monitor and evaluate the effectiveness of the education and training program through documentation evaluation and DRG validation review.

Laguna Medical Systems’ development team designed the AccuDOCUMENT Program specifically to meet the Hospital’s stated objective: to provide training and education to credentialed nurses and/or health information management professionals selected to transition into the role of Clinical Documentation Specialist.

LMS consultants will provide full-time education and training over the course of six to eight weeks, for a total of 240 to 320 classroom and on-site training hours.

LMS will provide a three-week (one week each 90 days) follow-up monitoring and program evaluation consisting of documentation review and DRG validation.
