The patient had a stricture of the urethral meatus. Assign 598.9, Urethral stricture, unspecified, to correctly code this diagnosis.

The surgeon performed a urethral meatotomy and an exploratory cystoscopy. According to the operative report, the meatotomy was not performed as part of the cystoscopy, but was a separate procedure. Assign ICD-9-CM codes 58.1, Urethral meatotomy, and 57.32, Other cystoscopy, for this procedure. Two CPT codes are also needed. Assign 53020, Meatotomy, cutting of meatus (separate procedure), except infant, and 52000-59, Cystourethroscopy (separate procedure), for this case. Modifier -59 is appropriately appended to 52000 to indicate that a distinct procedural service was performed as a separate component of the meatotomy procedure.
 

Report # 2

The patient had stress urinary incontinence secondary to intrinsic sphincter deficiency. Assign 599.82, Intrinsic (urethral) sphincter deficiency (ISD). Code the incontinence 625.6, Stress incontinence, female.

The patient underwent a laparoscopic retropubic bladder neck suspension. The correct ICD-9-CM code for this procedure is 59.5, Retropubic urethral suspension. The correct CPT procedure code is 51990, Laparoscopy, surgical; urethral suspension for stress incontinence.
