Using ICD-9-CM and CPT-4, assign codes for diagnoses and procedure(s) described in the discharge summaries below.

  1. A Type I diabetic patient with diabetic nephropathy was seen in an outpatient dialysis unit for hemodialysis. The physician also indicated chronic renal failure in his diagnostic statement.
     
  2. A patient was admitted with postpartum acute renal failure 2 days after delivering twins.
     
  3. A patient was admitted for acute tubular necrosis with benign essential hypertension.
     
  4. The physician's final diagnosis is documented as end-stage renal disease with malignant hypertension.
     
  5. A patient with established hypertensive renal sclerosis was admitted for acute renal failure and treated with dialysis.
