Procedure Practice
 

Assign the appropriate ICD-9-CM diagnosis and procedure codes to the following coding scenarios:
   

Example 1:
A patient was admitted in acute respiratory failure due to hypertensive congestive heart failure. The patient was intubated on day one of his hospital stay and maintained on IMV for the following 48 hours.
 
Example 2:
A patient was admitted with acute exacerbation of COPD. Oxygen supplementation was initiated, but by the second hospital day the patient was tiring, ABGs were deteriorating and oxygen saturation was only 89% on 4 liters of O2 via nasal cannula. The patient was intubated and mechanically ventilated for a total of 72 hours to treat his acute respiratory failure.
  
Example 3:
A patient was admitted and treated for acute pulmonary edema associated with left ventricular failure.
  
Example 4:
A firefighter was admitted and treated for acute pulmonary edema secondary to smoke inhalation suffered while combating a house fire.
 
Example 5:
A patient with chronic pulmonary fibrosis was admitted and treated for acute viral pneumonia and secondary acute pulmonary edema.
 
Example 6:
A patient suffered an acute anterior myocardial infarction complicated by respiratory failure. The patient was intubated and ventilated for 96 hours.
 
Example 7:
A patient was admitted for respiratory failure with progressive myasthenia gravis.
 
Example 8:
A patient was admitted for CHF and diuresed with IV Lasix. Initial chest x-ray showed moderate pleural effusion which resolved over the course of hospital admission. The patient also suffered from mitral valve regurgitation and aortic insufficiency.
  
Example 9:
A patient was admitted with a pleural effusion of unknown etiology. The effusion was treated with thoracentesis and the patient discharged to undergo further outpatient workup.
  
Example 10:
A patient was admitted and treated for acute respiratory failure associated with Legionnaires' disease. The patient also suffered from congestive cardiomyopathy with associated congestive heart failure and chronic pleural effusion. An emergency tracheostomy was performed upon admission and the patient was maintained on a mechanical ventilator for 14 days.
